New Patient Registration
We welcome enquiries from anyone living in the practice area wanting to register. We are contracted to provide general medical services for patients living within Central Stockton , Fairfield, Hartburn, Oxbridge, Grangefield, Newham Grange, Whitehouse Farm and Elmtree.
Our reception staff will be happy to guide you through the procedure and eligibility can be quickly confirmed from your address so please provide proof by way of a recent utility bill along with one form of photo ID.
You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. Both forms are available to download from the above "Registering online" tab.
All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.
Medical treatment is available from the date of registration. Please contact reception for further information.
Change of Name/Address/Telephone Number
If you change your name, address or telephone number please notify the receptionist as soon as possible. It is important that your records contain up to date accurate contact details.
If you are under the care of a hospital consultant, it is your responsibility to inform them of your change of address etc. Failure to do so may result in you missing an important appointment or hospital admission date.
If your new address is outside of our practice boundaries you will be asked to register with another doctor within that area.
